ABOUT LOS ANGELES COUNTY DEPARTMENT OF HEALTH SERVICES:

Through academic affiliations with the University of California, Los Angeles (UCLA), the University of Southern California (USC), and the Charles R.

Drew University of Medicine and Sciences (CDU), DHS hospitals are training sites for physicians completing their Graduate Medical Education in nearly every medical specialty and subspecialty.

In addition, to its direct clinical services, DHS also runs the Emergency Medical Services (EMS) Agency and the County's 911 emergency response system, as well as Housing for Health and the Office of Diversion and Re-entry, each with a critical role in connecting vulnerable populations, including those released from correctional and institutional settings, to supportive housing.

DEFINITION:

Performs health education assignments.


Essential Job Functions:


  • Coordinates daily health education clinical referrals and conducts individual, group, telehealth, and inperson health education classes for clients/patients clients by lecturing on and discussing various health topics including but not limited to hypertension, high cholesterol, weight management, prediabetes, and diabetes.
  • Implements the Chronic Disease Self-Management Program (CDSMP), a nationally recognized evidencebased curriculum designed to promote and improve health outcomes for adults living with chronic illness(es) by teaching skills and offering tools to promote selfcare beyond the health care system.
  • Provides technologic support and guidance to patients and clients for online video group education and facilitates virtual group education workshops.
  • Administers pre and posttests to health education program participants in order to determine the effectiveness of educational methods and material presented and assembles these and program data such as clinic attendance for review and analysis by Health Educators, Senior Health Educators or appropriate staff; Documents patient interactions and visits in a clear and concise way, communicates with others in the Care Team, and follows up with patients regularly.
  • Records and documents patient interactions and visits in ORCHID (Electronic Medical Record) and follows up with patients regularly; Coordinates with staff in the Care Team to provide patient care in the form of health education.
  • Contacts community organizations, public and private agencies, and professional groups to elicit their cooperation and participation in community health programs; Collaborates with their ACN facility
  • Enhanced Care Management (ECM) care team to support patient outreach and engagement activities.
  • Serves as an active member of the PCMH (Patient-Centered Medical Home) Team by attending monthly meetings and working collaboratively with the Team to provide patientcentered car.
  • Assists RN-Care Managers, Behavioral Health team members, clients, and patients with accessing reliable sources of health information, local community resources and social support services.
  • Writes flyers for dissemination; speaks to groups on healthrelated topics; arranges for special speakers and participates in health education activities as needed.

Requirements:


SELECTION REQUIREMENTS:


OPTION I:
Two (2) years of experience in the coordination, planning, or implementation of a health-related program.


OPTION II:
A bachelor's degree or higher from an accredited college or university with a major in health education.

_A major in health education is defined as:__ a curriculum which emphasizes topics such as the evaluation of health education needs, creation of health education lessons and the advocation for health education services (i.e. Health Sciences, Community Health, Public Health, Health and Physical Education, Nutrition Sciences, Exercise Science or Kinesiology)._


LICENSE:


A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.

PHYSICAL CLASS II
  • Light:

This class requires light physical effort that may include occasional light lifting to a 10-pound limit and some bending, stooping, or squatting.

Considerable ambulation may be involved.


DESIRABLE QUALIFICATIONS:

Additional credit will be given to applicants who possess the following desirable qualifications:

  • Additional years of experience beyond the Selection Requirements.
