    Community Mainstreaming is a non-profit human service agency committed to providing services to help people with intellectual and developmental disabilities lead meaningful productive lives within their community.

    We are seeking a Community Habilitation Specialist to join our team The Community Habilitation Specialist will provide the oversight and support necessary to enable the program participants to manage all aspects of their lives maturely and responsibly.

    Locations: Nassau & Suffolk County, NY

    Position: Full time
    Hours: Monday – Friday, 9 am to 3 pm (30-hour work week)
    Hourly Rate: $18.00

    Responsibilities:

    • provides meaningful instructions to the individuals to assist with skills related to living in the community
    • assists with activities of daily living, personal care, health care, community inclusion, relationship building and development of social and leisure skills
    • accesses community services and supports
    • develops independent community travel skills
    • meets with individual to discuss behavioral concerns, problems identified by the individuals and/or sensitive subjects requiring attention
    • encourages individual to identify potential outcomes and problems assistance with problem solving training and support
    • works with individual to appropriately communicate with outside entities such as Care Coordinator vendors, doctors, employment specialist, landlord, etc.
    • assists the individual in coordinating a variety of leisure and community activities

    Qualifications:

    • must have a HS Diploma or GED
    • ID/DD experience preferred
    • must have a Valid NYS Driver's License
    • must have own transportation with proof of insurance and registration
    • must be flexible – evenings, weekends, holidays
    • must be Computer Proficient
    • smartphone required
