Community Habilitation Specialist - White Plains, United States - Triumphant Hands

Description

• Teach life skills, provide supports, and facilitate exploration of new activities necessary to enhance participant's development per the individual's habilitation plan.
• Provide personal care assistance as needed to ensure client safety, health and hygiene.
• Accompany client to community classes and provide assistance as needed.
• Provide support in the community to ensure safety in parking lots and street crossings.
• Adhere to scheduled days and hours of services as assigned.
• Provide safe transportation as needed and approved.
• Complete all documentation as instructed, including, but not limited, to timesheets and daily records of services according to timeframes required by agency policy.
• Interact professionally with individual, broker, family and other providers to ensure the individual is receiving appropriate services.
• Attend training as required and/or as needed to enhance professional knowledge.
• Attend planning team meetings if requested by the individual or broker.
• Conduct self in a manner to maximize participant growth in accordance with all regulations and agency policies and procedures.

• Day shift
• Monday to Friday
• Weekends as needed
