The Care Coordinator is responsible for securing and sustaining linkages in the community to promote a positive outcome for youth and family.
Supporting the operations of the IBHS Program and ensuring required documentation is completed to meet all regulatory, contractual, and credentialing requirements; complying with operating policies and procedures established for the assigned function; communicating with appropriate personnel; responding to inquiries or requests for information; maintaining appropriate records and providing assigned reports.
This is aMonday Friday full-time position.
Requirements:
Proficient reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communicative skills; knowledge of IBHS program operations and related standards, regulations, and procedures, including documentation requirements; proficient PC skills.
Clean motor vehicle record required. Ability to function independently, manage time successfully in relation to job responsibilities, and ability to navigate various community settings.Experience:
A minimum of one (1) year of experience in the Child and Adolescent Service _System _Program (CASSP) normally required.
Knowledge of available community resources including concrete goods such as housing, food, rental, and utility assistance, etc. Knowledge of community systems including child welfare, juvenile justice, and community mental health.Education:
A B.S. or B.A. degree in a human service-related field preferred. Associate degree or equivalent credit hours required.
