    As an innovative company providing in-home care to patients recently discharged from a hospital setting, Welcome Home House Calls is seeking Nurse Practitioners and PAs who are passionate about patient health and helping individuals avoid medical complications after acute care. Our providers play a critical role in ensuring a smooth transition back home and return to daily activities for our patients, all while enjoying the one-on-one patient relationship unique to in-home care.

    At Welcome Home House Calls, we value provider independence and offer flexible hours and extremely competitive compensation. You can choose between per-visit pay for those working half-days or unique schedules, or per-day pay for those working a full shift

    You can work part-time, full-time, or anywhere in between, making your own schedule with our availability coordination system. Plus, after 3-6 months of on-the-job experience, full-time commitments and generous compensation packages may be available.

    • Direct Physician Support- Colleagues who are there for you, not just for clinical questions, but to handle everyday tasks, such as approving prescriptions and ordering diagnostics, so you can focus on direct patient care
    • RN Case Management- Always available for your patients, even when you are not, and there for you to help with coordination, questions, talking to patients/family-members, and to make sure your patients get everything they need.
