Responsibilities of the Provider:

Skills & Requirements of the Advanced Practice Provider (APP):

Compassion, empathy, and self-awareness
Ability to multitask, think critically, and find solutions to any problem
Strong leadership and effective communication skills
At least 12 years of experience as a PCP responsible for managing chronic & acute medical problems as well as initiating medications and other interventions to manage such problems
Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.
Care coordination with other providers, specialists, testing facilities, and agencies.
Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidencebased breast cancer screening every 2 years)
Educating patients on their health conditions, care plans, and treatments.

Required Qualifications

Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
Active, nonprobationary state Nurse Practitioner license from National certification in at least one of the following specialties: Family Nurse Practitioner, Adult Nurse Practitioner
Active DEA license
US Work Authorization

Preferred Qualifications

Experience in
primary care: internal medicine, geriatrics, or family medicine
Understands HCC (Hierarchical Condition Categories) documentation, ICD10 (International Classification of Diseases10) Coding, and Health Risk Assessments (HRAs).
Passion for teamwork and the opportunity to collaborate crossfunctionally
Desires to be a part of an innovative model focused on empiricallyguided population health
Bilingual proficiency in applicable areas
