Responsibilities of the Provider:
- Deliver quality, valuebased, outcomedriven preventive healthcare.
- Diagnose & treat acute and chronic medical problems
- Coordinate care with other providers and specialists, as needed
- Strive to improve health and happiness for patients and team on a daily basis
- Cultivate a climate of trust and compassion
- Comply with medical care regulations and safety standards
Skills & Requirements of the Advanced Practice Provider (APP):
- Compassion, empathy, and self-awareness
- Ability to multitask, think critically, and find solutions to any problem
- Strong leadership and effective communication skills
- At least 12 years of experience as a PCP responsible for managing chronic & acute medical problems as well as initiating medications and other interventions to manage such problems
- Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.
- Care coordination with other providers, specialists, testing facilities, and agencies.
- Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidencebased breast cancer screening every 2 years)
- Educating patients on their health conditions, care plans, and treatments.
Required Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- Active, nonprobationary state Nurse Practitioner license from National certification in at least one of the following specialties: Family Nurse Practitioner, Adult Nurse Practitioner
- Active DEA license
- US Work Authorization
Preferred Qualifications
- Experience in
primary care: internal medicine, geriatrics, or family medicine - Understands HCC (Hierarchical Condition Categories) documentation, ICD10 (International Classification of Diseases10) Coding, and Health Risk Assessments (HRAs).
- Passion for teamwork and the opportunity to collaborate crossfunctionally
- Desires to be a part of an innovative model focused on empiricallyguided population health
- Bilingual proficiency in applicable areas
Pay:
From $110,000.00 per year
