Responsibilities of the Provider:


  • Deliver quality, valuebased, outcomedriven preventive healthcare.
  • Diagnose & treat acute and chronic medical problems
  • Coordinate care with other providers and specialists, as needed
  • Strive to improve health and happiness for patients and team on a daily basis
  • Cultivate a climate of trust and compassion
  • Comply with medical care regulations and safety standards

Skills & Requirements of the Advanced Practice Provider (APP):

  • Compassion, empathy, and self-awareness
  • Ability to multitask, think critically, and find solutions to any problem
  • Strong leadership and effective communication skills
  • At least 12 years of experience as a PCP responsible for managing chronic & acute medical problems as well as initiating medications and other interventions to manage such problems
  • Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.
  • Care coordination with other providers, specialists, testing facilities, and agencies.
  • Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidencebased breast cancer screening every 2 years)
  • Educating patients on their health conditions, care plans, and treatments.

Required Qualifications

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • Active, nonprobationary state Nurse Practitioner license from National certification in at least one of the following specialties: Family Nurse Practitioner, Adult Nurse Practitioner
  • Active DEA license
  • US Work Authorization

Preferred Qualifications

  • Experience in
    primary care: internal medicine, geriatrics, or family medicine
  • Understands HCC (Hierarchical Condition Categories) documentation, ICD10 (International Classification of Diseases10) Coding, and Health Risk Assessments (HRAs).
  • Passion for teamwork and the opportunity to collaborate crossfunctionally
  • Desires to be a part of an innovative model focused on empiricallyguided population health
  • Bilingual proficiency in applicable areas

Pay:
From $110,000.00 per year
