At the Cardinal Cushing Centers, we support people with intellectual and developmental disabilities, providing possibilities and opportunities for growth and advancement.

Our programs for students, adults and community members include education, employment training, transition services, residential care, therapeutic and recreational services.


The Case Manager will support and evaluate individuals in a day habilitation setting participating as a member of an interdisciplinary team in the coordination of therapeutic services.


  • Manages daily operations of the Department toward the selfdetermination and achievement of individuals receiving supports and the assurance of their health and safety in all locations where individuals receive supports and services
  • Assists in the development of the intake and referral process in the residential department
  • Assures an organized and balanced flow of daily activities that meet the needs, goals, and interests of individuals receiving Department supports and those of the contracting agencies, companies, and businesses
  • Assures adaptations necessary for individuals to participate in home, community, and leisure activities
  • Develops, implements and evaluates innovative opportunities for people with disabilities to live, work and recreate in their communities within the Department's areas of responsibility
  • Assures Department promotion of and compliance with the principles and standards of CARF survey and certification.
  • Assures Department compliance with all local, state and federal rules and regulations, including but not limited MassHealth, DDS, and those relating to public health and labor.
  • Implements a continuous quality improvement program within the Department
  • Approves financial transactions within the Department in accordance with CCC guidelines
  • Maintains accurate records and files
  • Contributes to Annual Department Report
  • Oversees the maintenance and upkeep of building and vehicles operated through the Department

Case Load responsibilities:

  • Write monthly and quarterly progress notes, write ISP/DHSP's, write assessments and objectives for annual review. Assist with development of behavior plans or other intervention strategies. Chart data collected from program.
  • Share oversight of individual's case load in terms of treatment and habilitation monthly goals and inform staff& Site Director of any changes in the individual's status.
  • Participate in the Inter Disciplinary Team process for intake of new clients.
  • Refer all staffing issues to the Site Director or Senior Director of Day Services.
  • Supervise the implementation of DHSP and maintain the individual's records.
  • Coordinate OT, PT, Speech, Nursing and Behavioral services for the assigned individuals.
  • Assist individuals in making decisions about their daily routine/activities.
  • Assure that individuals have access to communication with family, friends, significant others.
  • Maintain appropriate certifications (i.e. First Aid, CPR, Safety Care, Fire Safety, Human Rights, etc.)
  • Other Duties as assigned
  • Knowledge and understanding of principles of normalization, behavior management and human rights.
Related Work Experience

  • Supervisory or managerial experience in the field of disabilities
  • Evidence of experience with persons with disabilities and their families
  • Evidence of experience directly related to the responsibilities of the Department
Formal Education, Certification, Licensing

  • Bachelor's degree in a human servicerelated field and must have at least one year of experience working in a program with individuals with intellectual disabilities.
  • Valid driver's license
