Hope For Youth has been awarded a contract to provide Prevention Services to Nassau County. Hope For Youth's Nassau Prevention Services is a program of time-limited, family-oriented, supportive and rehabilitative child welfare services, which are provided to children and their families for the purposes of:

Averting a disruption of a family that will or could result in the imminent placement of a child in foster care;
Enabling a child who has been placed in foster care to return to his/her family at an earlier time other than would otherwise be possible; or
Reducing the likelihood that a child who has been discharged from foster care would return to such care.

The Nassau Prevention Case Planner reports to the Program Supervisor.

To be a successful Case Planner you must be ready, willing and able to do the following always:

Function within Agency's policies and procedures as outlined in the Employee Manual.
Function within the agency policy and procedures as outlined in the program guide and applicable codes of conduct.
Provide support to the clients and families- i.e., the knowledge, skills and tools necessary in order for them to maintain the child in their home.
Achieve successful outcomes within the guidelines outlined in NYS, County and HFY policy and procedure (shared).
Perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope for Youth, oversight agency and any other regulatory obligations.

How you will generally spend your time in this job:

Maintaining regular contact with the youth and their families at least twice a week, one contact being in the home and implementing the various parts of the program model (including evenings, nights and weekends) for caseload
Completing the initial CANS-NY and all subsequent CANS-NY, as well as attending school meetings and court appearances with the families as necessary and be responsible for discharge/transition planning
Providing case management services, life coaching, parent skill training, and make all collateral contacts as necessary
Regularly reviewing the service plan with the youth and family; meeting with supervisor to review case record
Documentation and related paperwork
Staff meetings, treatment team meetings and conferences
Training and staff development
Other related assignments as needed to be successful

To qualify for this job you need to have at minimum:

BSW (or Bachelor's in a related field) plus 2 years related casework experience or MSW
Bilingual in English/Spanish
Excellent written, verbal and computer skills
Flexible schedule to accommodate clients and program
Valid NYS Driver's License
