Our South Bronx Team


We are expanding our work in the South Bronx We currently work with community members through the Bronx H.Y.P.E Family Enrichment Center.

Family Enrichment Centers (FECs) are warm and welcoming community gathering places where any family or individual can come for support, information, and to participate in and/or lead activities (called "offerings").

FECs are based on the premise that community members design and drive the FEC's activities or offerings.

We are excited to further our mission through the implementation of the school-based model in South Bronx public schools, specifically in districts 7 and 12.


Case Planner Position Description


The Case Planner is primarily responsible for providing services to 12 families in school districts 7 and 12 in the Bronx, including having a significant daily/weekly presence in up to 2 schools.

This role is ideal for someone who is passionate about equity and access to high quality educational and social services.

This position requires adaptability, patience, and a commitment to trauma-informed care. The Case Planner will report directly to a Program Supervisor.

Work Hours:
Full-Time; evening and weekend work may be required.


Specific responsibilities include but are not limited to:

  • Provides school-based services to a caseload of twelve (12) families;
  • Completes home visits, and family and group counseling;
  • Implements researchbased programs and practices, including Triple P and Motivational Interviewing (MI);
  • Uses advocacy, outreach and crisis intervention to support families;
  • Engage families immediately upon referral;
  • Cocreates case plans with families;
  • Listens to, empathizes, and connects with families;
  • Carries oneself with the highest level of professionalism;
  • Conducts screenings and assessments;
  • Works collaboratively and creatively to address the needs of all stakeholders;
  • Fosters a collaborative partnership with schools;
  • Travels to partner schools often, and maintains a regular presence at schools;
  • Participates in Family Team Conferences, school meetings, hearings, and other meetings concerning the wellbeing of families;
  • Plans and implements activities for families and partner schools;
  • Prepares and maintains FASP documentation, monthly statistical reports, and case work notes;
  • Manages and updates all documentation systems regularly;
  • Maintains the general appearance of their work area and collaborates with staff to ensure high standards of cleanliness, organization and beauty of the physical work environment;
  • Maintains flexibility, adaptability and poise at all times;
  • Performs other duties as assigned.

Minimum Qualifications

  • BA/BS/BSW;
  • Two years of relevant work experience;
  • Excellent oral and written communication skills;
  • Excellent time management and organization skills;
  • Excellent interpersonal skills;
  • The ability to travel within the South Bronx;
  • Available to work at least one (1) evening per week to accommodate family schedules when needed.

Preferred Qualifications

  • BSW preferred
  • Bilingual (Spanish and English);
  • At least three (3) years of experience living and/or working in the Bronx;
  • Experience working in NYC Public Schools.

Pay:
$50,000-$54,000 per year, based on experience and qualifications
