    We partner with several well-known and highly regarded government healthcare agencies, such as the Department on Behavioral Health, the Department on Disability Services, and the National Institutes of Health.

    Job Description


    This budget for this position is covered by a grant based with the Department of Behavioral Health (DBH) called the District of Columbia Opioid Response (DCOR).

    The Care Coordinators and Certified Peers workers act as a liaison between community programs, resources, and community members to assist individuals with OUD.

    Care Coordinators and Certified Peers will locate, engage and identify individuals who are very vulnerable and struggle with OUD, SUD, and/or have frequent ER visits due to overdoses and assist these individuals with emergency referral sources, and provide them with comprehensive case management services.

    Per the DCOR grant conditions, care coordination and certified peers must have experience in working with individuals who experience OUD, co-current disorder, SMI, and homelessness.


    Duties:

    • Must know how to maneuver through communities and conduct outreach and identify individuals with OUD.
    • Know how to network and build partnerships with community programs, emergency agencies, treatment facilities, government agencies, CSA, PSH providers, etc.
    • Understand and know how to complete a realistic individualized goals and care plan that is person-centered .
    • Provide information and referrals to community and government programs for emergency needs and other needs identified by the individuals, that interferes with engaging in services
    • Ensure individuals are connected to medical, mental health, substance abuse, and other services as appropriate .
    • Provided the individual with excellent case management services .
    • Must have organizational skills and can maneuver on the computer .
    • Complete documentation, monthly reports, and entering data in a timely manner
    • Understand the importance of collaborating with the individual's CSW, PSH provider, PCP, probation officer, etc. to ensure the individual continues to have a good support system
    • Weekly supervision and team meetings.
    Qualifications


    • Bachelor's degree or equivalent experience
    • 1-2 years of experience with individuals with OUD
    • 1-2 years experience working with individuals who are homeless .
    • Ability to work in a diverse environment .
    • Experience working with adults with behavioral health difficulties is required.
    • Knowledge of resources of emergency services and government agencies
    • Must be well-organized and detail-oriented .
    • Basic computer skills, including Microsoft Office
