SummaryProvides daily supportive and functional care to residents in a compassionate environment according to established community care procedures as directed by nursing leadership.
Works as a collaborative member of an interdisciplinary team that focuses on the resident as the director of their own care.
Job ResponsibilitiesMaintains residents' comfort; provides physical care with activities of daily living according to scope of practice; answers resident's call lights and requests; reports observations of residents to the nurseHandles residents' personal care needs and support in a timely manner; helps maintain residents' dignity by letting them do as much for themselves as possibleObserves residents closely to identify changing needs and conditions and reports information to the nurseActively participates in residents' care/service plans for the purpose of improving residents' livesAssists residents during mealtimes and feeds residents who need full assistanceCreates and maintains meaningful relationships with residents and their families; provides support with compassion and professionalismSupports residents' involvement in decision making regarding their own daily activities; Assists residents with activitiesAssists with residents' therapies and treatments under the direction of licensed staffDocuments care provided in clinical recordsMaintains a clean and safe environmentQualifications, Knowledge, Skills & AbilitiesCurrent CNA certificationA desire to be part of a person-centered care teamPrior experience with direct patient/resident care preferablePhysical Requirements and Working ConditionsThe physical activities, demands, and working conditions described here are representative of those an employee encounters while performing the essential functions of this job.
