- Makes the initial nursing evaluation visit promptly upon referral receipt, assessing and reassessing the patient's nursing needs, including physical and mental status assessments.
- Initiates and coordinates the treatment plan per psychiatric nursing practice standards.
- Instructs and supervises the patient's family in caring for the patient and maintaining a safe environment.
- Provides specialized nursing services, administers treatments ordered by the physician.
- Assists the patient's family in understanding daily living challenges and available community resources.
- Maintains accurate patient history records for supervisors and physicians.
- Reports changes in the patient's condition to the physician and relevant personnel.
- Offers counseling and crisis intervention to patients and families.
- Participates in in-service programs, nursing reports, and the Care Management Model.
- Performs record reviews, collaborates with healthcare team members, and attends staff conferences.
- Contributes to the QA/QI plan and process.
- Completes all assigned duties.
- Prior to RN psychiatric nurse visits, the agency will follow Psychiatric Skilled Nursing/Scope of Care guidelines.
- Recent experience within the last five years or evidence of psychiatric retraining classes or CEUs.
- Active RN licensure and current CPR certification.
- Valid driver's license, vehicle insurance, and access to reliable transportation.
