Personal Care Assistant


Duties:

• Provide personal care services to patients in accordance with their individual care plans
• Assist with activities of daily living, including bathing, dressing, grooming, and toileting
• Administer medications as prescribed by healthcare professionals
• Monitor and document patient's vital signs and report any changes or concerns to the appropriate healthcare professionals
• Assist with mobility and transfers, using proper body mechanics and assistive devices as needed
• Provide companionship and emotional support to patients
• Assist with meal planning, preparation, and feeding, including tube feeding if necessary
• Follow infection control procedures to maintain a clean and safe environment for patients
• Adhere to HIPAA regulations to ensure patient confidentiality

Skills:

• Experience in hospice care, dementia care, assisted living, or memory care is preferred
• Knowledge of medication administration and ability to follow medication schedules accurately
• Strong communication skills to effectively interact with patients, families, and healthcare professionals
• Ability to provide compassionate and patientcentered care
• Proficient in using medical equipment and systems for documentation purposes
