    *RN Case Manager opening at a hospital in Baltimore, MD *Looking for an RN with hospital discharge planning experience.

    *Requirements for RN Case Manager: * *MD or compact RN license
    • Must have 2+ years of inpatient discharge planning experience
    • CERNER experience
    *Pay & Perks of Nurse Case Manager: * *$48 per hour
    • Good work/life balance: Only 40 hours per week.
    • 13 week assignments give you the flexibility to see if the company is a good fit.
    • Possible temporary to permanent.

    * *Shifts:
    • M-F, 8-5p, weekend availability as needed
    *Position Objective:
    • Provides case management services to medicine/surgical teams. Completes an initial screen of all assigned patients to identify readmission risks, patient strengths and needs related to clinical resource utilization and transition planning.
    • Initiates, completes and documents transition planning for patients in assigned caseload. Collaborates with members of the health care team, including leadership of interdisciplinary rounds, to insure the multidisciplinary plan of care is developed, followed, modified as needed and coordinates the delivery of clinical services in order to insure that the patient's clinical, quality and cost outcomes are met. Refers cases with potential risk of non-coverages or discrepancy in admission status to the physician and to appropriate utilization management and administrative staff. Functions as a consultant to health care team members with clinical service and system problems within the specialty area educating staff and physicians about managed care principles, discharge planning, reimbursement, levels of care and the continuum of care. Attends and participates in relevant committee meetings such as Joint Practice Committee, Long stay review, Length of stay reduction work groups, staff meetings.
    • Manages patients based on departmental procedures and maintaining clinical safety and quality
    • Enters and maintains patient information in all departmental systems, per operational guidelines
