- Lead the implementation and ongoing assessment of patient care plans, collaborating with the spiritual care coordinator, social worker, hospice aide, and medical director
- Ensure thorough evaluation and development of a comprehensive care plan tailored to individual patient needs
- Maintain open communication regarding patient conditions and work alongside other healthcare providers to adapt care as needed
- Educational background: Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required; Bachelor of Science in Nursing (BSN) preferred
- Active license as a Registered Nurse (RN) in the related state
- Current CPR/First Aid certification
- Access to reliable transportation for commuting to care sites or work locations
- Minimum of one year of professional RN experience in a hospice or similar setting
