    Summary:

    This position will work with children and families involved with several JFCS programs, to include Adolescent Diversion & Assistance Program and Community Wellness Program. The goal is to provide family supports and referrals to resources that play integral roles in optimal family functioning.


    Key Functions of Position:

    • Provide linkage to resources, coordination of services and monitoring of progress to drive optimal family functioning.
    • Conduct client assessments and provide on-site services at community agencies, schools, courts, or any other location where the client may need advocacy or assistance.
    • Maintain case records in a timely and compliant manner.
    • Document client's progress and interventions in accordance with HIPAA guidelines and other federal and state regulations.
    • Provide advocacy for families in need of various resources.
    • Have knowledge of available resources in the service area for children and families.
    • Other reasonable duties as assigned.

    Education, Experience and Values:

    • Bachelor's Degree in Human Service related field from an accredited academic institution
    • At least 2 years' experience in casework or closely related work
    • Excellent written and oral communication skills
    • Experience working with youth from diverse populations and backgrounds
    • Well organized with excellent time management skills
    • Appreciates the cultural diversity of our community
    • Bilingual (English/Spanish) a plus
