- Provide linkage to resources, coordination of services and monitoring of progress to drive optimal family functioning.
- Conduct client assessments and provide on-site services at community agencies, schools, courts, or any other location where the client may need advocacy or assistance.
- Maintain case records in a timely and compliant manner.
- Document client's progress and interventions in accordance with HIPAA guidelines and other federal and state regulations.
- Provide advocacy for families in need of various resources.
- Have knowledge of available resources in the service area for children and families.
- Other reasonable duties as assigned.
- Bachelor's Degree in Human Service related field from an accredited academic institution
- At least 2 years' experience in casework or closely related work
- Excellent written and oral communication skills
- Experience working with youth from diverse populations and backgrounds
- Well organized with excellent time management skills
- Appreciates the cultural diversity of our community
- Bilingual (English/Spanish) a plus
