ABOUT THE ROLE:


Time-limited Subsidy is a Housing First program focused on providing solutions to unhoused youth by assisting them to move into permanent housing.

With the assistance of the Case Manager, transitional aged youth (TAY; ages receive services and resources to build life skills that support youth to meet their responsibilities.

The Case Manager will ensure that youth feel supported as tenants and empower them with the services needed to maintain permanent housing after they transition out of the program.

The Case Manager will help their clients increase life skills, obtain employment, and search for permanent housing.

The Case Manager will assist their clients obtain vital documentation, support through the rental search and move-in process, and provide individualized case management and supportive services to meet the needs of the youth.


WHAT YOU'LL DO:


  • Conduct housing searches and identify housing options suitable for youth and in compliance with program requirements.
  • Provide case management to connect youth to wraparound supports through SPY and other community resources.
  • Ongoing assessment of youth needs and developing individualized housing and service plans that are updated monthly in partnership with youth.
  • Provide crisis management and deescalation interventions as needed (e.g. minimize risk that could jeopardize the tenantlandlord relationship).
  • Engage youth in life skills workshops and activities (e.g. how to budget, utilizing public assistance benefits, accessing free or lowcost services, conflict resolution, etc).
  • Engage youth in increasing their income to ensure they are able to sustain market rate housing.
  • Refer and link youth to community services, such as education and employment opportunities, mental health resources, legal aid, etc.
  • Organize, schedule, and facilitate youth and team meetings.
  • Maintain youth files in compliance with program standards.
  • Submit documentation, notes and reports in a timely manner.
  • Attend all required meetings and trainings.
  • Build and maintain relationships with landlords and community agencies.
  • This position works on a 24hour site and you will need to work nights/weekends as needed.
  • Other duties as assigned to support the mission of the organization

Requirements:


WHAT YOU'LL NEED:


  • B.A. or B.S. in Psychology, Sociology, or related field preferred.
  • 12 years of experience in case management, counseling, or related experience is required.
  • 12 years of experience working with individuals experiencing or at risk of homelessness is preferred.
  • Experience working with youth preferred.
  • Individuals with lived experience of homelessness, incarceration, or addictions are strongly encoruaged to apply.
  • Valid California driver's license, personal vehicle and insurance

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:


  • Prolonged periods of sitting at a desk and working on a computer.
  • Changes in the environment, such as the office or outdoors.
  • Physical effort/lifting, such as sedentary

Mandated Reporting Requirements:

The person holding this position is considered a 'mandated reporter' under the California Child Abuse and Neglect Reporting Act and is required to comply with the requirements set forth in CSU Executive Order 1083 Revised July 21, 2017 as a condition of employment.


Driver Positions:


Applicants are required to provide a copy of their driving record from the Department of Motor Vehicles, along with a copy of their valid driver's license and proof of insurance.

Job offers are contingent upon a satisfactory driving record.
