How This Position Supports Our Vision

The Case Manager for Harvest House's Home Again supportive housing program provides coordinated advocacy, comprehensive case management and wrap-around support to assist families experiencing housing instability become and remain stably housed.

This involves regular assessments and ongoing progress monitoring of clients to provide accountability, including housing plans, goal setting, continuously updated service plans, budget planning, exit plans and any other necessary support to facilitate the objective of all clients obtaining permanent housing and achieving self-sufficiency.

This role requires compliance with all expectations of pertinent contracts, such as completing mandatory assessments and reports and maintaining documentation as required.


Key Responsibilities

  • Provide comprehensive case management services to all participants including: intake assessment, benefit assessment, goal setting, case plan development, progress monitoring, budget planning, advocacy and referrals.
  • Maintain a caseload of 20 families with children.
  • Meet with participants biweekly (one meeting in the office and one meeting in the home) to facilitate their achieving selfsufficiency: set goals, access community resources/support services and obtain permanent housing.
  • Complete unit inspections during monthly home visits to ensure units are well maintained.
  • Document all interactions with participants accurately and in a timely manner utilizing appropriate reporting mechanisms, including: uploading and maintaining accurate data required for the Homeless Management Information System (HMIS), locally known as Community Services Information System (CSIS).
  • Participate in regular staff meetings, staff training programs, supervisory sessions, and accept the responsibility for aiding the development of positive team relationships.
  • Adhere to agency policy, procedures, and the professional code of ethics.
  • Implement program expectations and assist in improving such expectations.
  • Maintain participant and program records in accordance with applicable standards, regulations, and grant requirements.
  • Maintain a high level of ethical conduct regarding confidentiality, dualrelationships, and professional stature.
  • Maintain flexibility in scheduling.
  • Other duties as assigned by supervisor.

Expectations and Commitments

  • Primarily M-F, 8 AM to 5 PM
  • Fulltime hourly position (40 hours/wk).
  • Valid driver's license and reliable transportation is necessary.
  • B.A. in the social service field.
  • A minimum of three years experience in case management.
  • Strong computer skills and the ability to learn new programs.
  • Knowledge of community resources.
  • Ability to work independently and as a team with a strong sense of focus, taskoriented, nonjudgmental, open personal qualities, clear sense of boundaries.
  • Excellent people skills with a welcoming, upbeat personality and the ability to communicate effectively.
  • Capable of maintaining accurate, timely case plans and notes.
  • Ability to function effectively under stressful situations.
  • Must maintain a professional approach to all aspects of work.
  • Ability to legally operate a motor vehicle and provide own transportation.
  • Possess compassion and hope for the individuals served.
  • Passionately dedicated to service.
  • Ability to problemsolve on the spot.
  • Bilingual is a plus.

Pay:
$ $24.00 per hour

Expected hours: 40 per week
