The Southern New Jersey Perinatal Cooperative (The Cooperative) is a private, nonprofit 501(c)(3) coalition of healthcare professionals and consumers licensed by the New Jersey Department of Health as the maternal and child health consortium serving the seven counties of South Jersey. Our biggest perk"Summer Flex" and Hybrid Work ScheduleThe Community Health Worker is responsible for conducting targeted community outreach activities to provide a source of enhanced social support and create a bridge between under-served and hard-to-reach populations and formal providers of health, social and other community services. This individual participates in community and educational activities to support the mission and goals of the Healthy Women Healthy Families Project.
Requirements:Participates in community meetings, events, and health fairs linking high-risk families to Central Intake for further referral. * Works closely with Referral Specialists. * Provides health education to teach individuals how to make good health decisions, incorporate healthy habits/activities, and avoid harmful behaviors. * Promotes health insurance/Medicaid enrollment. * Provides case management to women who are not eligible for referral to home visiting programs and are in need of support and services. * Networks with community partners in public health clinics, schools, colleges, hospitals, community centers, nonprofit organizations, and government agencies to increase referrals into Central Intake; assists with dissemination of HWHF materials in community through daily outreach. * Ensures confidentiality of all client information. * Participates in ongoing training sessions, conferences, and seminars. * Completes all documentation in SPECT system to collect data, and track daily activities and event contacts. * Attends and participates in appropriate Project meetings and activities, and other meetings as needed. * Performs other duties as assigned.
