***The Valley Mills Hospital is a specialized clinic for internal medicine with 122 beds and 10 day care places.

Our medical spectrum includes three departments with the following focuses: 1. Cardiology, intensive care medicine and geriatrics; 2. Pneumology, gastroenterology and sleep medicine; 3. Rheumatology.

Your responsibilities:

Participation in coordinated discharge and securing of aftercare for patients with poststationary care and support needs
Social legal advice
Collaboration with payers and care partners
Counseling tasks for psychosocial conflicts and crises of patients
Coordination of interfaces for social and vocational rehabilitation efforts
Preparation of social reports

Your profile:

Completed degree in a relevant field (preferably social pedagogy or a comparable qualification)
Experience in hospital social work
Knowledge of current social legislation and its application
Patientoriented selfconcept
Cooperative collaboration in an interdisciplinary team
Personal identification with the objectives of a Christian institution
