    Position Summary

    The Telemedicine Medical Assistant (Tele-MA) serves to facilitate community and home-based, real-time telehealth encounters and specimen collection for healthcare clinicians at a distance from Mary's Center's clinic sites. The Tele-MA acts as a liaison between healthcare clinicians, patients, and families. The Tele-MA will work with the clinical team to ensure patients receive quality care and access to care through telehealth and care coordination.

    This position requires travel to multiple community locations in Washington, DC, and Maryland (Montgomery and PG Counties). May be required to work evenings and weekends to meet the needs of patient or healthcare clinician.

    Reportability

    This position will report directly to the Nurse Manager of Facilitated Telemedicine.

    Essential Duties and Responsibilities

    To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

    • Facilitates telehealth consultations by completing pre-visit chart reviews and ensuring all equipment is operable prior to the visit; cleans and sterilizes equipment as needed, travels to and from the appointment sites, orients patients to the technology, facilitates communication between provider and patient as needed, documents encounter in EMR and participates in assessments by utilizing scopes and other specialized equipment for the purposes of the telehealth encounter.
    • Assists the healthcare clinicians in obtaining any necessary clinical patient information, such as lab results, diagnostic tests performed, hospital summaries imaging results, discharge summaries, and current medication.
    • Performs and documents screening procedures for completion of health evaluations, e.g. laboratory tests, hearing, and vision testing, geriatric assessments, growth charts, head circumference, height, weight, and vital signs.
    • Collects specimens, performs laboratory tests and/or phlebotomy as ordered by the provider and safely stores and transports all samples for lab drop off.
    • Administers vaccine and injections under medical supervision, per provider order.
    • Provides medical education, health promotion and discharge instructions to patients per order of the medical clinician.
    • Coordinates with the care team in developing and implementing a patient's plan of care through the use of motivational interviewing and care planning techniques and provides resources and support services for patients experiencing barriers to care, including internal referrals and linkages to care team members.
    • Manages a panel of My Health GPS patients and/or care coordination patients.
    • Complies with operational procedures & requirements (such as HIPAA guidelines, and agency mandates).
    • Performs other duties determined as necessary by the Director of Care Coordination and the Nurse Manager of Facilitated Telemedicine.

    Qualifications

    Knowledge, Skills & Abilities -

    • Minimum of High School Diploma or GED Equivalent.
    • At least 2 years of relevant experience in direct patient care
    • Medical Assistance (MA) certification or registration
    • Trained or certified (preferred) in phlebotomy ideally with recent experience
    • Experience working in community health settings with immigrant populations and/or maternal-child health preferred; must be willing to visit patients in shelter or shared living situations.
    • Must have a strong ability to utilize and troubleshoot technology
    • Experience working with culturally diverse and underserved populations.
    • Excellent organizational skills and attention to detail
    • Team player with the ability to work collaboratively with staff that works remotely
    • A thorough understanding and working knowledge of patient confidentiality issues and HIPAA compliance/regulations are essential

    Language Requirement - Ability to communicate effectively in English is required. Additional language proficiency or fluency in Spanish preferred.

    Physical Demands - Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear; must be able to lift objects up to twenty-five (25) pounds.

    Work Environment - Typical medical office or home/community setting with quiet to moderate noise level; travel distances up to 45 minutes from clinic sites = May be exposed to extreme cold, heat, and humidity due to outside weather conditions. May be exposed to potentially hazardous blood-borne/respiratory pathogens and bodily fluids which require the practice of Universal Precaution procedures.
