Who You Are:
As our Transitions of Care Coordinator
What You'll Do:
Prepare and assemble patient records to ensure completeness and accuracy. Obtain prior authorizations for referrals, medications, and diagnostic procedures. Coordinate referrals to specialty care and community partners, ensuring seamless transitions. Arrange transportation and escorts for patients to appointments and testing. Track referrals and diagnostic testing to verify success and adherence. Facilitate internal team referrals and participate in quality improvement plans.
What You'll Bring:
A high school diploma or GED equivalent is required, with an associate's or bachelor's degree preferred. Alternatively, you have 3-5 years of relevant healthcare experience. Experience working with vulnerable populations and a non-judgmental approach. Strong communication skills, both interpersonal and written. Excellent teamwork, organizational, and multitasking abilities. Proficiency in computer skills. Self-directed with a proactive approach to problem-solving.
