    JOB DESCRIPTION
    Summary: The Registered Nurse is an integral part of the clinical healthcare team, delivering essential patient care within a community health care setting. Responsibilities include providing nursing services, patient/family education, and managing the flow of patients with autonomy and sound judgment.

    Essential Functions:

    • Case Management
      • One-on-one chronic disease care management.
      • Development of comprehensive care plans.
      • Patient/family counseling and education.
      • Coordination of resources and referrals.
      • Advocacy for patient needs.
      • Facilitation of access to community resources.
    • Patient Education
      • Prescribed treatment regimen education.
      • Disease process education.
      • Healthcare system navigation guidance.
      • Development of patient educational materials.
      • Improving healthcare delivery.
    • Electronic Health Record (EHR)
      • Accurate documentation entry.
      • Pre-visit patient medical information updates.
      • Basic EHR understanding.
      • Appointment scheduling and workflow management.
      • Problem flowsheets troubleshooting.
    • Provider Support
      • Nursing policy adherence.
      • Assistance with procedures and EHR management.
      • Support for providers in electronic order entry.
      • Patient flow assistance.
    • QI/QA
      • Active participation in quality improvement projects.
      • Compliance with care standards.
      • Identification and intervention in quality issues.
    • Clerical Duties
      • Scheduling appointments.
      • Phone message taking.
      • Document scanning in EHR.
      • Patient registration.
      • Medical record pre-screening.
      • Document management support.
      • Patient referral assistance.
    • Leadership
      • Patient assessment and flow management.
      • Medication inventory management.
      • Sample and pharmacy log maintenance.
      • Medical supplies ordering and maintenance.
      • Report completion and other related duties.
      • Supervisory coverage as needed.
    Minimum Qualifications:
    • Graduate of an accredited nursing school.
    • Florida State RN License.
    • Effective communication skills.
    • Interaction ability with diverse stakeholders.
    • Proficiency in Office 365 programs.
    • Knowledge of proper office equipment use.
    • Electronic health record competence.
    Preferred Qualifications:
    • Two years of Nursing experience.
    • Bilingual communication skills.
    Physical Demands: The role involves standing, walking, hand usage, lifting weights up to 25 pounds.

    Work Environment: Medical clinic/office setting.

    Job Type: Full-time

    Pay: $37.00 per hour
