    Job Description

    The mission of the Division of Infectious Diseases at Mount Sinai Morningside and Mount Sinai West has been to offer high quality, expert care to all of our patients. One facet of our care is our Outpatient Parenteral Antimicrobial Therapy (OPAT) program which allows our patients to complete their intravenous antibiotics in the comfort of their own home while being monitored by us. We also provide seamless outpatient follow up for our General ID patients who are being discharged on oral antibiotics.

    This position is responsible for the performance of increasingly independent and complex tasks of a routine and standardized nature. It will include direct diagnostic and therapeutic patient care under Physician supervision. The provider will perform outpatient consultations for General ID patients and patients who might benefit from enrollment in our OPAT program. The PA will also provide follow-ups for General ID/OPAT patients discharged from the hospital.

    KEY FUNCTIONS

    Conducts 5 clinic sessions per week at the Mount Sinai Morningside Multispecialty Clinic, which will include the care of patients on intravenous antibiotics as well as general ID consults.

    Conducts one clinic session at the Mount Sinai Morningside FPA practice in conjunction with supervising physician that includes OPAT patient follow-up appointments as well as general ID consults.

    Responsible for working the different infusion companies (including but not limited to Optioncare and Mount Sinai Home Infusion) to review patient labs and coordinate the initiation of outpatient antibiotic therapy.

    One day per week, work with the inpatient consult teams at either Mount Sinai Morningside or Mount Sinai West (determined based on consult volume) to conduct inpatient ID consultations. This will help provide some continuity for the patients who are ultimately seen in our outpatient ID clinic.

    Provides assessment, triage, and therapeutic management of patient/family needs throughout the disease process in conjunction with the supervising physician.

    Acts under the authority and supervision of the attending physician and in accordance with regulations, hospital policy, protocols, and clinical pathways.

    Obtains medical histories, performs physical examinations, orders labs and radiographic studies and reviews results. Formulates and manages patient treatment plans and outcomes.

    Prescribes treatment and signs prescriptions in accordance with institution and state requirements.

    Educates patient/family on care and resources.

    Collaborates on research projects, when applicable.

    Supports access to the OPAT program for patients discharged from the hospital services.

    Performs outpatient consultations for patients whose primary teams would like for them to be enrolled in the OPAT program.

    Performs follow-up visits for outpatients enrolled in OPAT with problems while on therapy, performs follow-up visits for patients completing OPAT.

    Performs General Infectious Diseases outpatient consultations and follow ups.

    Discusses/coordinates patient care with relevant specialists to ensure optimal outcomes.

    EDUCATION

    Required: Master's degree from an accredited Physician Assistant (PA) program by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA), except for Physician Assistants who graduated from a PA program prior to 2013.

    EXPERIENCE

    Preferred: Prior experience as Physician Assistant.

    LICENSURE / CERTIFICATION

    Required: PA certification by the National Commission of Certification of Physician Assistants. Licensed by New York State

    MAY BE REQUIRED: Eligible to be granted limited prescriptive authority by the State of New York
