This role involves collaborating with medical and dental staff to enhance patient outcomes and care.
- Assess Chronic Care, adherence, outcomes, and initiatives.
- Evaluate medical and dental needs promptly.
- Create and manage comprehensive care plans.
- Ensure compliance with operational policies.
- Engage with Chronic Care patients and monitor outcomes.
- Attend patient case conferences.
- Act as a liaison for patients with external entities.
- Maintain up-to-date Chronic Care statistics.
- Stay informed on industry trends and methods.
- Deliver professional and courteous service.
- H.S. Diploma/GED
- 2 years of healthcare experience.
- Proficiency in MS Word, Excel, PowerPoint, and Outlook.
- Strong verbal and written communication.
- Ability to handle confidential matters with discretion.
- Clear and respectful communication skills.
- 2 years as a Patient Services Coordinator.
- Experience with medical software such as Centricity.
- Knowledge of electronic health records like Athena Health.
