    Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.

    Benefits:
    We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit.     This position is included in a bargaining unit of Housing Works' New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union ("RWDSU"). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a "CBA") between Housing Works and the RWDSU

    This position requires applicants to successfully complete a background check through the New York State Office of Children and Family Services when it supports the mandate under Section 424-a of the Social Services Law for legally authorized agencies to inquire of the Statewide Central Register of Child Abuse and Maltreatment (SCR) as to the existence of any indicated reports of child abuse and maltreatment against an applicant prior to employment, certification, or licensure in the childcare field. All background checks will be conducted and reviewed in accordance with applicable federal, state and local law.

    Compensation Range: $22/hr - $24/hr commensurate with experience

    Overview
    :

    The Care Manager will implement effective prevention strategies intended to serve New York City's most at-risk queer communities with a focus on TGNC (Transgender & Gender Non-Conforming) & MSM (men that have sex with men) communities of color. In this dynamic position the Care Manager will provide all aspects of care coordination for SAMHSA participants from enrollment to discharge. This includes completing GRPA Intake, follow-up, and discharge assessments, developing care plans, and coordinating provider appointments. The Care Manager assists clients with accessing services, including but not limited to primary care, substance use treatment, mental health counseling, housing, and vocational/educational resources. The Care Manager also assists with recruitment and implementation of evidence-based program interventions such as Anger Management & Seeking Safety. This position includes field work: visiting the client's homes, areas in the community where clients want to meet, other HW sites, and participation in community and event-based outreach.

    Responsibilities:
    • Provide supportive case management to transgender, gender non-conforming individuals, and MSM of color at high risk for substance abuse and/or co-occurring mental health conditions.
    • Conduct GRPA Intake, Follow-up, and Discharge Assessments
    • Provide care coordination on an ongoing basis and participate in integrated case conferences to support treatment adherence and improved health outcomes
    • Follow up with clients and providers to ensure that appointments are kept, patients are engaged in care, and referred to needed treatment.
    • Engage with clients via social media for outreach, re-engagement or case management follow up activities in compliance with HIPPA protocol.
    • Maintain essential data for reporting and program evaluation in eICare, including program enrollments, progress notes, care plans, and referral activity.
    • Build and maintain a resource lists for external services and emergency services (i.e., housing, domestic violence, food pantry)
    • Coordinate with outreach staff to identify and address presenting issues of new enrollees.
    • Facilitate presentations for internal and external programs to promote the program services for the purpose of garnering new program enrollees.
    • Participate in weekly supervisory sessions with the Program Coordinator
    • Participate in monthly chart review as well as CQI site meetings and programmatic projects.
    • Ensure successfulreferral/linkageto psychosocial services andproviders including referrals via warm hand-off.
    • Participates in daily huddleswithclinic staff to discuss clientneeds and coordinate care as needed.
    • Work in collaboration with multidisciplinary team membersto address clinical barriers,including eternal partners.
    • Perform various administrative duties, including updating client trackers and outreach logs; assisting with outreach calls, and scheduling intakes and follow ups as needed.
    • Attend Interdisciplinary Team meetings as the program representative, and update info by due date in preparation for meetings.
    • Perform other duties as assignedby supervisor
    Minimum Requirements
    • Bachelor's Degree in related field preferred, or HS diploma/GED combined with 3 years' work experience in related field of human services
    • Minimum of 2 years of case management experience in a related setting
    • Strong verbal and written communications skills
    • Good problem-solving skills, ability to work autonomously, and meet deadlines
    • Knowledge of HIV/AIDS, homeless, trauma, substance use and mental illness, and LGBT issues preferred
    • Strong organizational skills
    • Professional experience with social media, dating apps and other digital marketing tools is preferred with a willingness to develop skills further
