DESCRIPTION OF DUTIES / ESSENTIAL FUNCTIONS

Screens immunization records for uptodate status.
Conducts data entry in the Texas Immunization Registry (ImmTrac2) to ensure uptodate shot records.
Provides and documents service linkage that connects client families to internal and external service resources.
Gathers and collects information to generate reports, activity logs, etc.
May act as a backup to a customer service section and/or administrative support services
Conducts community projects, presentations and other outreach activities to educate clients and families on the benefits of the Project Milestone program.
Other duties, as assigned
Performs data entry activities to record information.
May distribute forms to customers to complete for City services.
May refer customers who require additional assistance to the appropriate agencies.
This position may require valid Drives License.

WORKING CONDITIONS

The position is physically comfortable most of the time with occasional periods of stooping, bending and/or light lifting of materials of up to 10 pounds.

MINIMUM REQUIREMENTS:

MINIMUM EDUCATION REQUIREMENTS
Requires a high school diploma or a GED.

MINIMUM EXPERIENCE REQUIREMENTS

Six (6) months of administrative or customer service related experience is required.

Substitution:
An Associate's degree may be substituted for the experience requirement.

MINIMUM LICENSE REQUIREMENTS
None.
