Compensation: $64.70 to $72.46 per hour.


Location - San Jose, CA, $20,000 Sign-On Bonus.


Full-Time Position with Rotating Weekends

Position Summary


Are you a compassionate leader ready to make a difference? Join our team as the Manager of Social Services In this rewarding role, you'll oversee the operations of the Social Services Department, working collaboratively to provide top-notch care to our patients.


Your Responsibilities
-
Leadership: Report to the Director of Case Management and lead the department's operations.

Policy Development: Create and implement policies, procedures, and quality improvement programs.

Budget Management: Develop and manage capital and operating budgets within your areas of responsibility.

Team Building: Ensure your department is appropriately staffed by hiring and retaining competent staff.
-
Performance Excellence: Drive quality and performance improvement activities in the department and multidisciplinary teams.
-
Patient-Centered Care: Strive for positive customer satisfaction results by delivering exceptional care.

Compliance: Ensure compliance with federal, state, and local regulations and guidelines, including JCAHO, Medicare, CMS, and more.

Relationship Building: Foster positive, productive relationships with physicians and colleagues.


Your Qualifications
-
Education: Hold a Master's Degree in Social Work (MSW).
-
License/Certifications: Preferably be a Licensed Clinical Social Worker (LCSW).
-
Experience: Have at least 2 years of general acute care experience, with expertise in Social Services, acute care management, trauma/stroke, and homelessness case management/discharging planning.
