    Job Summary & Deliverables


    We are here to help provide support on the path to recovery through evidence based group therapy and recovery oriented treatment, supportive case management and medication management including MAT services.


    The Social Service Worker provides therapeutic behavior management, case management, and other services as defined by licensure to ensure excellence in client care, fiscal management, and regulatory compliance.

    Essential Functions

    Provides therapeutic behavior management as prescribed by care plan
    Assists therapist as needed to develop and review safety/crisis plans and care plans
    Develops therapeutic relationships with clients
    Reviews and assesses care plans
    Coordinates services among internal/external agencies and internal/external providers
    Follows care plan to monitor progress, teach skills, address behavioral concerns, and bill accordingly
    Ensures continuity of care

    Meets required productivity standards and other organizational expectations

    Requirements

    Bachelor's degree in Social Work or related field
    Internship experience as required for SSW license
    SSW licensure in state of UTAH | DOPL Verified
    CPR certification | Valley can provide

    Valley de-escalation certification
