    Job Summary & Deliverables

    South Valley outpatient services provide a full array substance use and behavioral treatment services to adults. The philosophy is that there is relief in rediscovery, opportunity, and responsibility in recovery. Through support, education, practice, and desire to change.     We are here to help provide support on the path to recovery through evidence based group therapy and recovery oriented treatment, supportive case management and medication management including MAT services.

    The Social Service Worker provides t herapeutic behavior management, case management, and other services as defined by licensure to ensure excellence in client care, fiscal management, and regulatory compliance.

    Essential Functions
    • Provides therapeutic behavior management as prescribed by care plan
    • Assists therapist as needed to develop and review safety/crisis plans and care plans
    • Develops therapeutic relationships with clients
    • Reviews and assesses care plans
    • Coordinates services among internal/external agencies and internal/external providers
    • Follows care plan to monitor progress, teach skills, address behavioral concerns, and bill accordingly
    • Ensures continuity of care
    • Meets required productivity standards and other organizational expectations

    Requirements
    • Bachelor's degree in Social Work or related field
    • Internship experience as required for SSW license
    • SSW licensure in state of UTAH | DOPL Verified
    • CPR certification | Valley can provide
    • Valley de-escalation certification

    Range Min: $23/hr calculated upon experience
