Supervisor:
Birmingham Area Regional Director
Summary of Job Duties:
The SOS Coordinator works with the Regional Director to conduct the coordination of care between all stakeholders, including the coordination of care meetings.
This position will be in the Birmingham/Tuscaloosa, Alabama area.Specific Responsibilities:
- Must work a designated number of hours each week
- Participate in didactic trainings and additional trainings as required
- Provide systemic consultation
- Participate in Project Transition trainings and events
- Maintain linkages and relationships with community partners
- Assist with referrals for consultation and treatment as needed
- Ensure the coordination of SOS team meetings and crisis plan development for individuals
- Provide outreach visits to the individuals' home, day and vocational settings as needed
- Provide Support and training on the crisis plan to both member and members SOS team
- Develop Comprehensive Assessments, Cross-Systems Crisis Prevention and Intervention Plans, intake/assessments, intervention and outcome plans any other applicable documentation of services provided
- Maintain an active caseload of individuals referred for SOS Services
- Provide oncall support on a rotational basis as part of a SOS Team
- Complete trainings as aligned with regulatory compliance
- Other duties as assigned

Qualifications
- A Master's Degree in Social Work, Psychology, Counseling or other human service field with 2years' experience working with people who have IDD and mental health and/or behavioral health needs
- Bachelor's Degree in Social Work, Psychology, Counseling or other human services with 5years' experience working with people who have IDD and mental health and/or challenging behavior needs is acceptable.
- The ability to work with complex systems and an interest in the population is essential
- Independent and organized work skills along with communication and writing skills are necessary.
- Experience working with families and individuals
- Must have driver's license and reliable automobile
