    Title: SOS Masters Level Intern

    Supervisor: National Director of Outpatient Services

    Summary of Job Duties:

    The SOS Coordinator Intern works with the National Director of Outpatient Services and SOS coordinators to learn the concepts of coordination of care between all stakeholders, including the coordination of care meetings.

    Specific Responsibilities:

    • Must work a designated number of hours each week required by the University
    • Maintain all requirements from the University for the field placement
    • Participate in Project Transition trainings and events
    • Learn and participate in developing community linkages
    • Learn and participate in providing initial services and assessments for new members
    • Participate in and learn to facilitate the coordination of SOS team meetings and crisis plan development for individuals
    • Learn to and facilitate needed outreach visits to the individuals' home, day and vocational settings
    • Learn to develop and understand the Comprehensive Assessments, Cross-Systems Crisis Prevention and Intervention Plans, intake/assessments, intervention and outcome plans any other applicable documentation of services provided
    • Complete trainings as aligned with regulatory compliance
    • Other duties as comfort and clinical capabilities develop

    Qualifications

    • Bachelor's Degree in Social Work, Psychology, Counseling or other human services
    • Actively enrolled in a Master's level program in a related field
    • Recommended: Experience working with families and individuals
