    Job Description

    Assignment Level III

    Under administrative direction, with the widest latitude for the exercise of independent initiative and judgment, supervises a complex Pharmacy unit or multiple Pharmacy units and/or performs as a specialty clinical pharmacist in a highly-specialized field within a hospital, clinic or other health care setting. In addition to performing the duties of Assignment Levels I and II at a more responsible level, the following are typical tasks performed at Assignment Level III:

    Examples of Typical Tasks:
    1. Initiates and/or participates in research studies conducted in the specialty area; conducts drug administration aspects of drug research projects and prepares reports of findings.
    2. Provides pharmaceutical education to professional staff at lectures, meetings or individually.
    3. Coordinates the activities of other supervisors in integrating the functions and staff activity of multiple Pharmacy units.
    4. Antimicrobial Stewardship Program development includes but is not limited to: IV-to-PO Conversion; automatic therapeutic substitution; therapy de-escalation; pharmacist intervention monitoring and reporting.
    5. Monitors, evaluates and reports on Adverse Drug Reactions (ADRs), Medication Errors and other aspects of infectious disease care as appropriate.
    6. Develops and conducts orientation/in-service training related to Antimicrobial Stewardship Programs for all pharmacy & nursing personnel
    7. Utilizing needs-based assessment, develops and conducts multi-disciplinary educational programs and services.
    8. Ensures all documentation requirements are fulfilled and submitted on or before deadlines, including but not limited to staff competency assessments and Quality Improvement Reports.
    9. Demonstrates knowledge of the department's role in fulfilling the hospital's mission.
    10. Supervises the activities of pharmacy personnel to insure the accurate completion of assigned tasks.
    11. Maintains accurate and complete records and statistics of departmental activities.
    12. Participates in inter-departmental activities, committees, meetings and other functions as required.
    13. Submits electronic health record generated antibiotic utilization reports to NHSN on a monthly basis.
    14. Provide Pharmacist coverage as requested by the Director or his / her designee.

    Minimum Qualifications

    For Level I:
    1. Possession of a valid NY State license to practice pharmacy.

    For Level II: In addition to meeting the qualification requirements of the lower level:
    1. Two years of post licensure experience; or
    2. Completion of a post graduate specialty residency or fellowship program; or
    3. Current Pharmacy Board Certification.

    For Level III: In addition to meeting the qualification requirements of the lower levels, an additional:
    1. One year of post licensure supervisory experience or specialty clinical experience.
