We are here because you matter And we believe that every child matters too Safe Kids, Strong Families ESSENTIAL FUNCTIONSImplement the Healthy Families model and curriculum with fidelity and follow all program and agency policies and procedures.
Conduct visits in a family's home and maintain a caseload of 15-25 families.Attend and participate in required virtual and/or in-person trainings and program meetings.
Build and maintain relationships with community agencies and partners to share program information and learn about resources available for families.
Participate in Continuous Quality Improvement activities.Administer screenings for parents and children.
Completes weekly recruitment strategies within the programs service area, in an effort to reach eligible families.
Supports the eligibility process for interested families including the collection and analysis of required documentation when necessaryModels and support parents to enhance parent-child relationships and support parents' role as their child's first teacher and lifelong educator.
Supports families by completing family assessments, encouraging family goals and providing community resources and referrals that support family well-being.Completes ongoing case notes documentation, required forms and reporting for families on caseload.
Coordinates and participates in all family engagement activities such as parent meetings, workshops and events.
Completes additional assignments, tasks, and reports as directedQualifications Education/ Experience:
