Zoraida, a Spanish-speaking nurse, was often the only nurse able to provide care in her patient's preferred language. Admissions Nurse

Note about Pay:

Pay is calculated per visit at $42-$45 per visit, depending on experience.

Start of care is paid at $150 per visit


Overview:

The Admissions Nurse is responsible for assessing, developing, implementing, and evaluating nursing care for assigned patients.

Admissions RN must visit the patient and complete a comprehensive assessment of the patient's/family's medical/psychosocial status, nutritional needs and safety.

The Admissions RN establishes a plan of treatment which includes the various services required to maintain the patient safely in the home or other residence(s).

The Plan of care includes quantifiable goals based on desired outcomes.


Position Responsibilities:


  • Create an initial assessment of patients and confirm existing data.
  • Create and implement nursing care plans and schedules.
  • Coordinate the admission process, including completing necessary paperwork and obtaining requited authorizations.
  • Receive, review, and upload the admission documents, patient consents and followup with
  • CoPs.
  • Responsible for the development, implementation, and updates of the individualized plan of
- care.

  • Ensure each patient and care plan has an individualized documented emergency preparedness plan.
  • Collaborate with physicians, case managers, and other healthcare professionals.
  • Assign, instruct, supervise and evaluate home health aide care.
  • Comply with the discharge planning requirements in conjunction with the patient at admission.
  • Conduct comprehensive health assessments to evaluate the patient's medical history, current condition, and specific care needs. Ensure patient needs are continually assessed and addressed in the plan of care, that the PoC is delivered in a timely and effective manner and that the goals of care are achieved. It must include patient and caregiver education and training to facilitate timely discharge, patientspecific interventions and education, and measurable outcomes and goals identified by the agency and the patient, as a Condition of Participation.
  • Perform medication reconciliations and ensure proper medication management. Ensure all medication lock boxes located in the patient home have the company information label and copy of their medication list.
  • Ensure that all necessary medical equipment and supplies are ordered and in place for the patient's care.
  • Educate patients and their families about the home healthcare program, and other available services and expectations
  • Communicate effectively with the interdisciplinary team to ensure seamless care coordination.
  • Maintain accurate and uptodate patient records and documentation in compliance with regulatory standards.
  • Participate in ongoing training and professional development
  • Ensure all services that will be provided to the patient are reasonable, necessary, and based
- solely on the patient's unique condition and need.

  • Maintain departmental policies, procedures, objectives, and infection control standards.
  • Adhere to all company policies, procedures, and ethical standards.
