    At 4th Second, we are committed to catalyzing systemic change and fostering collaboration to bridge gaps between community needs and resources.

    Focused on BIPOC populations in Solano County, including youth, people experiencing homelessness, people who have been justice impacted, and individuals with low incomes, our programs include street medicine, health care coordination, youth substance use and suicide prevention,

    whole person workforce development, and community development to support food and housing sovereignty.


    Position Overview:
    We are seeking a dynamic and dedicated individual to join our team as the Equity and Outreach Manager.

    In this key role, you will be responsible for conducting assessments of the current outreach strategy and health equity initiatives, and utilizing the PATH CITED technical assistance marketplace to develop infrastructure to support 4th Second's Cal-Aim expansion.

    The position will play a crucial role in developing and implementing outreach and equity strategies to ensure the organization's programs expand equitable access to health care, address social determinants of health, and create workforce pathways to support individuals with lived experiences or homelessness, incarceration, substance use to become service providers for our Whole Person Care service expansion.


    Key Responsibilities:

    Outreach and Equity Assessment:
    Conduct an assessment of the current outreach strategy, utilizing the TA marketplace.
    Evaluate the existing strategy to address health equity, utilizing the TA marketplace.
    Collaborate with community partners and internal teams to gather insights and feedback on current outreach initiatives.

    Strategy Development:
    Develop a comprehensive outreach strategy that aligns with Cal-AIM requirements and addresses factors of health equity.
    Utilize findings from the assessments to inform the development of equitable outreach, data visualization, and communication strategies.

    Collaboration and Knowledge Building:
    Work closely with community partners to build knowledge and implement outreach systems that effectively engage priority populations.

    Engage with the CITED TA Marketplace, collaborative planning initiatives, and county-level collaborative meetings to contribute to equitable strategies in Cal-Aim workforce development and care access expansion.


    Health Equity Focus:
    Collaborate with the Cal-Aim Workforce Assessment Manager to integrate health equity considerations into workforce development and care delivery plans.

    Support the development of initiatives to ensure equity in outreach efforts, addressing the needs of marginalized groups, including BIPOC, LGBTQ2S+, justice-impacted populations.

    Collaborate with the organization's documentation and data teams to ensure data and stories center the voices of clients to support equity in reporting and communication.


    Qualifications:
    Bachelor's degree in Public Health, Social Sciences, or a related field.
    Experience in DEIA,

    outreach strategy development and health equity initiatives.
    Knowledge of health equity in the context of CAL-Aim.
    Familiarity with electronic outreach tracking systems.
    Excellent interpersonal and communication skills.
    Individuals with lived experience of homelessness or justice involvement are encouraged to apply.


    Salary and
    Benefits:


    Annual Salary:
    $80,000

    Full time of 35 hours per week
