- Drive to Senior Living Communities to deliver on-site primary care to residents
- Support the patient's care plan: managing chronic conditions, providing advanced treatments, and assisting with end-of-life care
- Collaborate with the interdisciplinary team, oversee medication management, and request lab tests
- Maintain clear and consistent communication with Physicians, office support personnel, community staff, and patients' families
- Hold a Certified Nurse Practitioner / CNP license in the State of Minnesota
- Prior experience in Geriatrics and/or Primary Care
- Possess a valid driver's license and current auto insurance
- Demonstrate a strong desire to serve the senior population
- Exhibit excellent interpersonal communication skills
