- Perform Adult/Geriatric Assessments, diagnose and treat medical conditions, offer health education, make physician referrals, and ensure clear documentation according to Matrix guidelines
- Collaborate with healthcare professionals, family members, and caregivers
- Work in various settings including home visits, skilled facility visits, and telehealth visits
- Collaborate with Primary Care Physician (PCP) for patient education and follow-up
- Master's Degree or equivalent experience and NP licensure
- Current RN and NP or PA licensure with prescription authority
- Board certified in a specialty approved by Matrix
- BLS, ACLS or CPR certification required
- Minimum 1 year experience as Nurse Practitioner/Physician Assistant
- Proficient in EMR software and strong communication skills
- Positive, outgoing, and adaptable
- Valid driver's license and access to a car with adequate insurance
- Travel to patient residences may be required

Key Responsibilities
What to Expect as a Matrix Provider:
Nurse Practitioner/Physician Assistant - PRN/Part Time - Home Risk Assessments
Type: PRN/Part Time - Flexible-Nurse Practitioner
Payment: $90.00 or $115.00 per completed Assessment based on county
Location: West Seneca, New York - Home, SNF, Telehealth, and other community environments
Hours: Flexible scheduling including days, evenings, nights, and weekends
Benefits: PRN/Part Time roles include 401(k) and limited Voluntary Benefits, plus Mileage Reimbursement plan
