    Description
    Job Summary:
    Deliver nursing care to a specific group of patients following Kaiser Foundation Health Plan of Washington ambulatory Nursing Care Standards, Nursing Code of Ethics, and Kaiser Foundation Health Plan of Washington policies. Coordinate and facilitate Health Care services for patients across different sections of the Health Care system. Collaborate with the Health Care team to provide and coordinate nursing care for patients, actively participating in patient assessment and care plan implementation.

    Essential Responsibilities:
    • Continuously assess the nursing care needs of a defined patient group using the nursing process and nursing diagnosis. Develop a comprehensive care plan for patient needs. Assist patients and families in setting health care goals as part of the care plan. Determine care priorities based on essential patient needs and available resources. Safely provide nursing interventions and offer necessary information to patients and families before procedures. Evaluate patient responses to care consistently. Anticipate changes in patients' conditions and modify care plans in collaboration with the health care team.
    • Demonstrate an understanding of the physiological and psychological effects of aging through appropriate care for different age groups. Communicate effectively with specific age populations served. Engage in educational programs related to specific patient age groups. Collaborate with other health care team members in planning and executing care. Assess the needs of patients requiring hospitalization, initiate nursing admission procedures, and coordinate discharge planning with inpatient staff and physicians. Maintain effective communication within the team to manage workloads efficiently.
    • Recognize and address the psychosocial, cultural, and emotional needs of patients and families in every nurse-patient interaction. Educate patients and families about the role of nurses and other health care professionals, available nursing services, and access to these services. Identify patients' educational requirements during nursing assessments. Develop educational strategies in each care plan. Provide appropriate teaching and counseling based on patients' and families' learning needs and medical care plans. Encourage patient compliance with treatment regimens. Base teaching on scientific and behavioral knowledge.
    • Assess patient lifestyle risk factors and incorporate health promotion and preventive measures into nursing care. Collaborate with patients to enhance their overall health status. Implement preventive measures, both physical and psychological, to support patient well-being. Promote patient self-care and responsibility for health needs. Adhere to documentation, safety, and infection control standards. Provide a safe environment for patients in accordance with safety protocols.
    • Participate in ongoing education programs to address personal learning needs. Contribute to staff orientation and student education initiatives. Work with managers to complete self and team performance evaluations. Ensure quality care by establishing, implementing, and evaluating professional nursing standards. Engage in multidisciplinary quality assurance activities and professional development. Adhere to personnel policies and maintain high professional standards. Maintain a professional appearance in line with current dress code policies.

    • Associates degree in Nursing from an accredited program.
    License, Certification, Registration
    • Registered Nurse License (Washington) required at hire
    • Basic Life Support required at hire

    • Strong communication, problem-solving, leadership, and decision-making skills.

    • Experience in ambulatory care nursing
    • Experience in primary nursing or case management
    • Bachelor of Science in Nursing (BSN)
