    Position Details:


    We are seeking a skilled and compassionate Cognitive Wellness Provider who specializes in conducting assessments and creating personalized care plans for individuals experiencing cognitive challenges.

    As a part of our team, you will play a vital role in improving the quality of life for our clients by accurately assessing their cognitive status and developing tailored care plans to enhance their overall well-being.


    Responsibilities:
    Conduct comprehensive cognitive assessments to evaluate memory, attention, problem-solving skills, and other cognitive functions.
    Analyze assessment results to identify strengths, weaknesses, and areas of improvement for each patient/resident.

    Collaborate with caregivers, and other healthcare professionals when needed and develop individualized care plans tailored to the patient's (resident) specific needs and goals.

    Monitor and track the progress of each patient /resident through regular assessments and adjustments to the care plan as needed.


    Requirements:
    Provider (NP license)
    Licensed in at least one of the following states; TX, IN, FL, PA
    Experience working with individuals with cognitive impairments, dementia, or related conditions.
    Strong interpersonal and communication skills to collaborate effectively with clients and multidisciplinary teams.
    Empathy, patience, and a genuine interest in promoting cognitive wellness and independence in others.
    Ability to create detailed and individualized care plans based on assessment findings
    Proficiency in using technology for assessment, documentation, and communication purposes.
    Commitment to ongoing professional development and learning in the field of cognitive wellness.


    Salary Range:
    $88 per hour
