    The Public Health Nurseis an essential team member who provides public health nursing services to promote, improve, or maintain health and well-being for families, children, and adolescent populations in Dakota County through family health programming.

    This role does not provide direct care in a clinic setting.

    Location & Schedule


    This hybrid position offers a mix of telework, in-person work at our Western Service Center in Apple Valley, MN, and work with community throughout Dakota County.

    Work is performed Monday through Friday during regular business hours (8:00 a.m. to 4:30 p.m.). Occasional evening and weekend community outreach opportunities may be available.

    What You'll Do


    As Public Health Nurse, you will conduct nursing assessments of family needs and assist with connection to resources via in-person visits, virtually or phone.

    You will create and plan engaging evidence-informed health-related educational outreach activities and materials for children, adolescents, and families.

    You will provide outreach services and trainings to community partners, residents of Dakota County, and health professionals about preventative medical and oral health, and a variety of health-related topics.

    Outreach opportunities may include in-person events, safety camps, and monthly individual phone calls as a part of the Child and Teen Checkups program.

    This role does not provide direct care in a clinic setting.


    This position will also work with our Intake program using critical thinking and attention to detail to assess and process referrals for family health programming using multiple computer software programs.

    You may also work with family health programs such as Child Passenger safety, Lead Case Management, Follow Along, or the oral health initiative to name a fewYou will participate in emergency response activities as outlined in the department emergency response plan and other duties as assigned.

    Minimum Qualifications


    NEED TO HAVE QUALIFICATIONS

    • Bachelor's Degree in Nursing AND
    • Current licensure as a Registered Nurse in Minnesota AND
    • Current licensure or qualifications to register as a Public Health Nurse in Minnesota AND
    • Valid driver's license AND
    • CPR certification

    NICE TO HAVE QUALIFICATIONS:

    • Professional nursing experience in acute care or community-based settings
    • Child Passenger Safety Technician Certification
    • Experience with women's and/or children's health
    • Experience with early childhood development
    • Experience with event organization and planning
    • Experience engaging diverse populations
    • Experience in creating engaging health-related activities or information for youth
    • Ability to read, write, and/or speak in a second language (e.g., Somali, Spanish)
    Duties & Responsibilities

    Regular attendance according to the position's management approved work schedule is required for all positions.


    • Conduct nursing assessments of family needs and assist with connection to resources.

    • Work with Family Health Intake to process referrals for family health programming using multiple computer software systems.
    • Work with other Family Health programs, such as Asthma, Lead, Follow Along, and Oral Health.
    • Participate in local workgroups and committees as assigned.
    • Perform public health emergency response functions as trained and assigned within the department's emergency response plan.
    • Work to advance inclusion, diversity, equity, and access in collaboration with colleagues and Dakota County residents.
    • Participate in strategic planning activities that support the department and Community Services Division's goal to create and sustain an integrated model for service delivery.
    • Perform other duties as assigned by the Family Health Supervisor.

    WORK ENVIRONMENT


    Work is performed in a hybrid model via telework, within a County office building, on client home visits and within the community.

    Travel is required.

    Exposure to hazardous physical conditions such as environmental conditions of unsanitary homes and property, unsafe neighborhoods, challenging and potentially violent and unpredictable clients and family members, pets, and animals; exposure to atmospheric conditions such as chemicals, odors, dusts, gases, poor ventilation, and cigarette smoke; exposure to hazardous materials such as chemicals, blood, and other body fluids; extreme temperatures; inadequate lighting; intense noise, and work space restricting movement may occur on visits to client's homes.

    Lifting requirement of up to 50 pounds on an occasional basis. May require lifting of up to 100 pounds on an infrequent basis.

    Equipment used may include, but is not limited to, computer, printer, telephone, calculator, copy and fax machines, TV/VCR, video camera, nursing supplies and equipment including scales, syringes, hazardous waste, masks, blood pressure equipment and car seats.
