    Description
    Summary: A key aspect of this role involves supporting clients in enhancing their physical, social, emotional, and daily life skills, boosting their independence for better integration into the community.
    • Assisting adults in care by providing crisis intervention, behavior management, life skills training, and vocational training, either one-on-one or in group settings.
    • Planning, implementing, and evaluating recreational, social, and educational activities for clients, fostering a fun and engaging environment to promote participation in programs.
    Reports to: CSA Clinical Manager and Clinical Director on matters related to patient treatment within Life Care Inc. programs.
    Workers Supervised: NONE
    Interrelationship: Requires excellent communication skills to relay essential information to various stakeholders such as Human Resources, Medical Director, and other relevant parties, ensuring alignment with the organization's mission and vision.
    Key Responsibilities:
    • Ensure full compliance with documentation requirements for consumers, including treatment plans, authorizations, and crisis plans.
    • Reduce consumer readmissions to hospitals, probation, and incarceration, by providing support in clients' homes and communities to enhance their independence and safety.
    • Identify and coordinate social, economic, recreational, and educational services that align with clients' individualized treatment plans.
    • Handle crisis and emergency situations effectively, maintaining the well-being of clients, staff, and the public.
    • Support the case management team in problem identification and program evaluation, offering recommendations for plan modifications.
    • Provide emotional and practical assistance to clients and their families, acting as a positive role model and delivering necessary life skills training.
    • Participate in program planning, develop tailored activity plans, monitor client progress, and adjust programs as needed.
    • Stay updated on activities programming trends, offer lay counseling support, and maintain positive relationships with clients and staff.
    • Assist with daily tasks such as client transportation, therapeutic activities, office operations, and meal preparation.
    • Ensure cleanliness and maintenance of facilities, including performing minor repairs and housekeeping duties.
    • Respond to inquiries, develop nutritious meal plans, assist with personal hygiene, and adhere to safety and health standards.
    • Administer medication according to protocols and handle other related duties as assigned.
    Education: High School Diploma/ GED/ BS in Behavioral Science/ 1-2 years' experience
