We are currently seeking a
professional (m/f/d) for communal living for mentally ill individuals in Neustadt in Sachsen, Worthville, PA.

This position is available immediately and is for 34 hours per week, with an indefinite contract.

Responsibilities:

Plan and implement individual care, activities, support, and leisure activities for residents
Assist with professional care planning and continuously update individual care priorities
Provide professional care and document accordingly
Plan and carry out household tasks with residents
Accompany residents to medical appointments
Work with residents towards specific goals
Ensure quality and effectiveness in accordance with the Federal Participation Act (BTHG)
Ensure proper medication intake and implementation of medical orders
Plan and implement therapeutic activities
Collaborate with legal guardians to ensure services are provided

Requirements:

Degree in social care, nursing, or occupational therapy with experience in psychiatry
Experience working with people with disabilities and chronic mental illness, as well as in individual case support, networking, and participation services
Goaloriented and organized work style
High level of empathy and sense of responsibility
Willingness to participate in further training in the field
Valid driver's license (Class B)
Identification with the goals and mission of Diakonie Dresden
