    Description
    WORK WITH COMPASSION

    KenCCID provides residential services for Individuals with developmental and intellectual disabilities at 30 community residences throughout Northeast Philadelphia. KenCCID is seeking to hire a Licensed Nurse Practitioner (LPN) to work to support the health and safety of our Individuals. This includes home visits, collaborating with physicians, pharmacists, and program staff, and other persons who have a direct impact on the Individuals' care requirements. Completes applicable reports/documentation when required. The incumbent collaborates and communicates with the nursing team and reports to the Director of Nursing.

    Position Responsibilities
    • Ensures that the safety and well-being of the Individuals is protected at all times and that the rights of the Individuals are observed at all times
    • Promotes patient's independence by establishing patient care goals; teaching the KenCCID team, Individuals, families and other stakeholders to understand condition, medications, and self-care skills; and responding to any questions
    • Establishes a compassionate environment by providing emotional, physical, and psychological support to Individuals and families
    • Collaborates with Director of Nursing, PD, MPSs, CHMs and DCPs to exchange information regarding Individuals' care, condition, and all matters affecting Individuals' health and wellbeing
    • Performs home visits to follow-up on Individuals after doctors' visit, when necessary
    • Train staff on medical areas/needs such as (i.e. Diabetes education, use of glucometer, patient transfer mechanism, blood pressure monitoring, etc.) and other applicable training
    • Schedules Individuals' doctor appointments, when necessary
    • Reports abnormal findings to the physician, CHMs, RDs, PD and Director of Nursing
    • Check medication logs at the end of each month, to ensure that they are in compliance with DPW 6400 Regulations
    • Ensure that the Individuals' dietary needs are met
    • Oversee and manage all post-operative care plans and perform treatments at the Individuals' homes as directed by the physician (e.g. bandage change)
    • Monitor and verify that the DCPs are following the doctors instructions for the best interest of the Individuals
    • Maintain and update the Individuals' Medical History summaries as required
    • Performs other related duties as required
    • These duties are subject to sudden change
    Education and Experience Requirements
    • LPN License
    • Driver's License
    • COVID-19 vaccination
    • Practical Nursing training from accredited Nursing Institution and 2 years of applicable experience
    • Knowledge of general healthcare and specific medical practices regulated by the DPW 6400 Regulations
    • Computer literate
