    About the Job:

    This position involves crucial responsibilities related to connecting HIV-positive individuals to medical care and maintaining their treatment. As a key member of the team at Esperanza Health Center (EHC), you will play a vital role in raising awareness about the HIV services offered by EHC and developing strong relationships with external social service agencies.

    Job Description:

    SUMMARY: Responsible for finding and connecting HIV-positive individuals to medical care and maintaining current patients in medical treatment, keep up to date with HIV-related issues and services, especially in EHC service area, building awareness in the community about the HIV services provided by EHC, and establish and maintains relationships with outside social service agencies and programs that can refer their clients to care at EHC. Provides support to Esperanza's social services team and population of patients with HIV by ensuring coordinated care and communication with external providers.

    DUTIES AND RESPONSIBILITIES:

    Outreach Duties

    • Develop and maintain strong relationships with community health, social support, and substance abuse treatment agencies.
    • Find and refer HIV+ clients to medical care.
    • Participate in community events such as health and resource fairs.
    • Conduct off-site and on-site HIV testing as needed.
    • Coordinate HIV testing promotions at various locations.
    • Collaborate with centers and shelters for HIV testing and medical care referrals.

    Social Services Duties

    • Review and update Case Management Plans for Ryan White Patients annually.
    • Monitor services renewals and expiries for patients.
    • Assist in scheduling specialty care appointments and requesting funding.
    • Support patients in accessing dental care and other medical services.
    • Coordinate transportation services for HIV patients.
    • Organize patient satisfaction surveys and support groups.
    • Respond to patient messages and assist in clinic testing.

    SPIRITUAL CARE:

    • Provide spiritual support and participate in spiritual growth activities.
    • Communicate hope and encouragement to patients through interactions.

    EDUCATION, KNOWLEDGE, SKILLS, AND QUALIFICATIONS:

    • Training in oral communication and at least one year of speaking experience.
    • High School Diploma.
    • Bilingual in Spanish and English.
    • Strong interpersonal and organizational skills.
    • Computer literacy and ability to handle confidential information.
    • Familiarity with HIV-related issues and community services.
    • Ability to lead discussions and teach.
    • Capable of forming trusted relationships with consumers.
