POSITIONSUMMARY: Responsible for day to day supervision ofcare coordination and population health programs. Responsible for advancing the functionality and reportingefficiency of a variety of Alternative Payment programs includingbut not limited to the following: CHQCA and community providerpractices including training on HEDIS measures and annual qualityabstraction to support (Medicare Shared Savings Program).
This position is responsible for the oversight of the MayoClinic E-consult, E-boards data support. Enhanced Oncology CareModel CMS, Transition of Care Programs, PCMH, VBP via cloud baseddata applications. Manager is responsible for compilation andpreparation of data results related to all aspects of carecoordination operations including; ACO care gap closures, CMS datametrics and documentation that supports contracted alternativepayment models including; CHRISTUS Health Plan, BCBS, PresbyterianCentennial, Optum, United Health Care STARS program and others.Manager provides supervision of accurate and timely billing fornon-face to face services for CMS program work.
RequirementsMINIMUMQUALIFICATIONS:
EDUCATION: Bachelors ofScience in Nursing required.
CERTIFICATION/LICENSES: Current license to practice as a RN inNew Mexico
SKILLS:
EXPERIENCE: Minimallyfive years supervisory experience in diversified positions within ahealth care organization. Considerable knowledge of populationhealth and alternative payment models preferred.

ENVIRONMENT:
PHYSICALREQUIREMENTS: Subject to stressfulprofessional relationships. Working hours vary, withflexibility due to unexpected changes in schedule andemergencies.
