- Assessing nursing home residents through interviews and evaluations to identify strengths and support their transition to the community.
- Creating personalized service plans and assisting residents in achieving their transition goals.
- Developing life skills, identifying necessary services, and advocating for all essential support for a safe transition.
- Facilitating discharge planning and community transition.
- Providing ongoing care management after community transition, coordinating benefits, scheduling appointments, and advocating for clients.
- Conducting motivational interviews for harm reduction plans and crisis interventions.
- Master's Degree (MA, MSW) in social work, counseling, or related fields for Social Services Professionals.
- RN or LPN certification for Nurses.
- LCSW or LCPC preferred.
- 1+ year of social services experience required.
- Experience with clients having mental health conditions or substance abuse preferred.
- Strong communication skills, computer proficiency, and community services knowledge.
- Sensitivity to cultural diversity and the ability to work with varied needs.
- Willingness to travel independently around Chicagoland area 50-60% of the work week.
- Experience with care management programs and consent decrees a plus.
- 35-hour work week, Monday-Friday with few exceptions.

AgeOptions has fulfilling positions for Social Service professionals with advanced degrees and licensing (M.S.W. or M.A., LSW, LCSW) and nursing professionals (L.P.N./R.N.) in their empowering H.O.P.E. Program. This program helps individuals shift from long-term care to living independently in the community. As part of interdisciplinary teams, you'll work in the office, field, and remotely (even from home), engaging in fieldwork 2-3 days weekly (traveling in and around the greater Chicago area).

About the H.O.P.E. Program
The Home Options Path to Empowerment (H.O.P.E.) Program, managed by AgeOptions, is a nursing home transition program funded by the Department of Human Services under the Colbert and Williams Consent Decrees. It focuses on aiding eligible nursing facility residents in transitioning to independent community living. The program uses interdisciplinary teams to provide holistic support, life skills training, emotional and physical care, ongoing case management for a safe and person-centered transition.
