The Oaks at Denville is searching for an experienced Social Worker to join our team
The Oaks at Denville is a Springpoint Senior Living community located in Denville, NJ.

We are in search of a Per Diem Social Worker to assist the Social Services department, and to provide medically related social services in order to attain & maintain the highest practicable physical, mental, and psychosocial wellbeing of each resident.

Responsibilities:

Assess the social/emotional needs of each resident within an appropriate time frame.
Perform documentation duties as required.
Interview residents/families as necessary in a confidential, private setting.
Involve the resident/family in planning social services programs.
Refer residents/families to other social service programs/agencies where appropriate.
Maintain a professional working relationship with medical representatives and other health related facilities and organizations.
Participate in community planning related to the interests of the facility and the services and needs of the residents and families.
Work with consultants as necessary and implement recommended changes as required or as appropriate which must be reviewed with the Director of Social Services prior to decision.
Ensure the emotional, educational, social, and spiritual needs of the residents are met.
Assist the Director of Social Services and/or the Admissions Coordinator with the adjustment period for new residents to the community.
Work with the Resident Review Team to ensure that each resident's needs are met.

Education and Experience:

BSW or MSW degree required.
Must be a CSW, LSW, or LCSW in the state of NJ.
Experience in Long Term Care setting preferred.
