Description

A New Jersey licensed social worker LSW/CSW is required for a newly opened Adult Daycare center in Essex County.
Responsibilities;
Documenting assessments, treatment plans, evaluations, and clinical
notes as per N.J.A.C guidelines
Coordinating discharge planning for participants, which shall be in
compliance with N.J.A.C.
Interviewing the participant and caregiver to obtain a social assessment
Providing or arranging for individual, family, and group counseling in
reference to psychological, social, financial, legal, vocational, and educational
needs of the participant
Monitor and evaluate clients' progress and modify treatment plans accordingly

Actively involve in community outreach program to help in growing the business.
