    Job DescriptionEssential Job Functions

    Foster Care Social Worker accomplishes the following:

    1. Visits foster homes on a pre-approved schedule according to the DCF contract to provide support and supervision of children in placement on assigned caseload. Documentation of services and support provided.
    1. Completes re-approval evaluations and monthly safety inspections.
    1. In conjunction with the Foster Care Supervisor and Foster Care Clinical Coordinator ensures that DCF agency licensing standards are maintained.
    1. Attends and / or facilitates ACR, OPS, PPT and all other relevant meetings for assigned caseloads.
    1. Ensures foster home providers receive ongoing training.
    1. Assures that client's finances are administered in an appropriate manner and that accurate records are maintained. Reports any discrepancies to the Associate Executive Director immediately.
    1. Responsible for on-call in the Foster Care program for crisis management and problem solving on an arranged schedule.
    1. In conjunction with the foster home providers, maintains an effective relationship with the client's families, guardians and friends.
    1. Maintains files on each client and provider.
    1. Maintains and increases professional knowledge and skills by attending training and staff development activities and keeping current by reading applicable professional literature.
    1. Interacts and relays information to team members including DCF, medical, nurses and school personnel.
    1. Other duties as assigned.
    Job Requirements
    1. Education: A Bachelor's Degree in Social Work and at least two years of experience in a child placing agency or a Bachelor's Degree in another related area of study and at least three years of experience in a child placing agency.
    1. Good leadership skills.
    1. Knowledge of Foster Home Placement Policies and Regulations.
    1. Good written and oral communication.
    1. Good organizational and planning skills.
    1. Commitment to the philosophy of community based services, integration and the values of client centered services.
    1. A safe driving record and a valid CT. driver's license.
    1. Suitable transportation.
    1. Residence located within appropriate distance for crisis response.
